504 Plan

Purpose: Ensures students with chronic illnesses like SCD can access learning in a general education setting.

Eligibility: Any student (K–college) with a health impairment that affects major life activities.


Accommodations:

Extra time for assignments/tests

Access to water bottles

Second set of textbooks at home

Modified physical activity (e.g., indoor recess)

Flexible attendance policies


Process:

Caregiver submits a written request to the school principal.

Include medical documentation from a healthcare provider.

School may conduct its own evaluation.


Review: Can be updated anytime; annual reviews encouraged.


IEP (Individualized Education Plan)

Purpose: Provides specialized education programming and services.

Eligibility: K–12 students whose academic needs require special education.

Services:

Customized teaching methods

Speech, physical, or occupational therapy

Assistive technology

Psychological counseling


Process:

Multidisciplinary team evaluation

Caregiver provides medical documentation

School assesses learning style and health needs

Review: Annual review required; caregivers can request changes anytime.


College Considerations

504 Plans continue into college.

Students must provide documentation.

Accommodations may include:

Extended test time

Priority registration

Note-taking assistance
